The 2025 fiscal year brought widespread performance gains for the nation's ten largest nonprofit health systems, though a significant expansion of their core business did not always follow. While operating revenues climbed across the board, the financial health of these massive organizations showed a mixed picture of growth versus stability.
According to recent financial filings, the leading systems all reported stronger operating results compared to the previous year. This improvement was largely driven by higher patient volumes, better reimbursement rates, and continued cost-control measures. However, for several of these giants, the increase in revenue did not translate into a proportional expansion of their hospital networks or service lines.
Some systems saw their operating margins improve by several percentage points, a welcome shift after years of thin margins and inflationary pressure. Yet, the data reveals that growth in patient admissions and outpatient visits was modest for many. Instead of building new facilities or acquiring smaller hospitals, several of the top systems focused on optimizing their existing operations and paying down debt.
The list is dominated by well-known Catholic and secular health systems, with the largest player reporting over $30 billion in operating revenue. While the top spot remained unchanged, there was some shuffling among the middle ranks as systems with aggressive expansion strategies in previous years paused to consolidate their gains. the 2025 results suggest that for the largest nonprofits, financial discipline and operational efficiency have become just as important as getting bigger.
